Appeals Process For Un-Approved Prior Authorization Requests

If you wish to appeal a denial of coverage based on prior authorization review, simply fax an appeal to US-Rx Care.

Note: Appeals must be submitted in writing and cannot be processed via phone.

As part of your appeal, please set forth the reason(s) you believe that the medication requested should not have been denied, include a summary of the pertinent facts relating to your appeal and include copies of any documents, records or other material that you believe are material and important for the review of your claim that were not previously provided to US-Rx Care.

Please reference the denial notice received from US-Rx Care for pertinent requirements for approval or approved alternatives for consideration before submitting an appeal. For medications not covered under the patient’s health plan, appeal requests cannot change the coverage status.
